is refilling common?

Anyone know if refilled vials are common? FOr example, a real bottle refilled with a fake substance or just sterile oil?

I'm sure it has happened. But not common. Most all users know that a new vial should ahve a flip cap.

I've know guys at the gym that would get gear for other people. Before giving it to them they would take a 1-2ml.
