I need help with a client waiver

Hello,

I would like to start taking pictures of my clients, before and after, but I would like to make a waiver for them to sign, I have no idea as to what needs to be said or how to say it.

I need this to cover people who are under the age of 18 as well.

Exactly what kind of waiver are you wanting? Knowing exactly what you want to cover will help you know what to put in it. Do you want one that says you are not responsible for injuries?

With the minor one, you may just want to have the parents sign something that they give you consent to train __________ (insert name) and maybe include a set period of time in there as well. Also may run it by an attorney. If a kids health goes downhill or gets injured you won't have anything to worry about that way.

no no, I have waiver to train.. thats not a problem I want one to take pictures of them so I can do before and after..but I know for a fact that I need some type of consent for a minor...as for others im not sure..and I want it to cover me in anyway possible

here's the dealio,
make up a waiver that says something to the fact of:
I___________________ (so and so client) give permission to__________________( thats you) to use and reproduce my LIKENESS (thats a photo video or any other media) then write down what you intend to use it for. have them sign, and the parent sign. easy as pie.
