Gyno Surgery Question

I'm currently in the process of going to the doctors. If I get a doctors referal to insurance to cover my surgery. What will that entitle?? Does insurance only cover cutting out the glands? For example the plastic surgeon will cut out the glands(covered by insurance) Leaving me to pay the difference to get the Lippo done?? If anyone wouldn't mind explaining this to me I'd appreciate it.
