Blood work pre-cycle

I might be going to get my "levels" tested If I can get my insurance to kick in. Not sure if they even pay for it.

What do I ask for? (before my first cycle) I guess I have to see an Endocronologist. correct? Or just a lab that takes blood? I`ve never done this before, in case they ask, what do you tell them? or more importantly Which tests do I get?

Is there a urine test too? I`m sure when I call the office they`ll ask "What tests do you want?" uhhhh
