since the bulk of code is in php pages, all they need to do is point textpad or another solid text editor like that and do a search on the highest level directory that holds them and look for that phrase.
that will turn up likely an error page that has all the various codes - see which one is getting triggered and what would cause it, and then see what page is calling it.
then see what circumstances cause it.

I can tell you it has nothing to do with cookies, and some people have the problem, some don't.
