Glutes are much more involved in lockout than upper back.
He also may have missed not because of weakness, but due to the fact that meets are very draining. I've missed a lift for my thrid deadlift attempt after smoking a weight for the second that was only 10 lbs lighter.

I second the no creatine...I'm 185 4 weeks out and just cut it...I'd rather lose the benefit of creatine, than the benefit of nutrition and hydration the last few days before....
That seems like a lot of weight on box squats....are you sitting back, relaxing your hips, using a low enough box, and exploding up?

It really depends on your current diet. If you drink everything but water, make the switch to nothing but water.....that can take you there by itself....increasing your fiber intake naturally or with psyllium husks is also something to try, as well as taking 2 doses of stacker or something...
