BTW. Totally forgot about ACE inhibitors effects on kidney function. While they are not nephrotoxic, they can reduce renal function.
So a blood test for creatinine before and after startup is necessary. If creatinine rises by 30 within a week then your kidneys are so fucked you cannot antagonize angiotensin-2 effects on renal arterioles. So neither can you use angiotensin-2 antagonists.
But this happens in only 5-10% of people treated that has hypertension and renal disease.
Other than this I think ACE inhibitors are amazing for treating hypertension IMHO. I've used ramipril with no side effects at all.
Older people(seniors) may experience dry coughing. This is not seen with AT-2 antagonists. Which do the same but by receptor blocking not hindering synthesis of AT-2.
