Milk is actually a relatively low gi substance. I can honestly say I've never heard of spiking insulin to take creatine other then PWO. Maybe somebody else can chime in, but I've always taken creatine in the past with a meal on days I don't lift. Days I lift I take it in my PWO shake.
But, if you did want to take it with a high gi carb id stick with the malto/dextro
