Whats up bro! you can get the form at the dermatologist office and I am by far under hardship.lol if I was I would not be able to afford gear. this is what I put down on the form I just stated I did not have a job and no insurance and yes it was that simple.they make you wait like 3 weeks for it . they send it in sample packs and not those tight ass one's at the pharmacy there is 100 tabs in each sample pack....my insurance recently expired and does not pick up for another month due to I quit my job a little while ago and have a new one now... call ROCHE pharmacuticals and ask about it if you dont believe me and I lied a little on it about my income but they did not seem to give a fuck....PEACE
