i used to do a lot of business in barcelona and madrid.
i can tell you first hand that the spanish dont like to learn foreign languages.. and their patience dealing with foreigners is ultra thin.
the menality, of some, is that if you cant speak spanish they dont wanna deal with you. and...
very few spoke fluent english so its a big problem.
those mandatory spanish courses in high school sure paid off though.
what about in the parts of spain that are heavily populated like the costa del sol etc, I might be headed out there soon and I'm interested in availability.
Go to portugal. Every pharmacy there has a chick who speaks english, they are friendly, and just make up a story that you forgot to bring your meds with you. Be sure to know the therapeutic doses.
