too many hand injuries

Hi guys I don't know if this is common or not or if somebody has gone through the same but since I started lifting, specially benching ( 4 months ago) I' ve had 2 thumb injuries 3 wrist injuries, they all affected my tendons and ligaments and I has to stop working out for a few weeks, what am I doing wrong guys? thank you.
 
There are two possibilities here - either there is something very wrong with your form, or you have some underlying condition. This is not normal.
 
Something is very wrong with your grip. That is not normal at all.

Book an appointment with a personal trainer and have him expect your form. You are doing something wrong. You should not be hurting yourself when working out.
