Looking for advice

Hey all. I am 31, 270 lbs and a drive a truck for a living. My weight has started causing some health issues( blood pressure, joint pain) so I've decided that it's time to fix it. I am able to get to the gym 3-4 times a week. I'm about three weeks in so I'm still doing some pretty basic stuff: 20 minutes of cardio and a few different workouts on the cable machines. I just read RADARs grocery list post so I'll be changing my diet presently.
So my questions are: where do I go from here? What should I add/change at the gym? Should I be taking workout supplements?

Any help would be appreciated.
